1.  I need to change a provider's demographic information (address, phone, etc.)
 If the provider is registered at Humana Military's MyHMHS for Providers section of the Humana Military website (www.humana-military.com), the provider can make changes in their own demographic information. To register today, go to the MyHMHS for Providers section at www.humana-military.com.

3.  My provider started using a new Tax ID number.
 The "fix" for this issue depends on whether or not the new Tax ID is on file with HMHS. If not, please contact your Provider Relations Representative (PERR). If you have already done this, you can add the new Tax ID to provider access list by click the link below. You must be signed in to add a provider id. You will need to keep the "old" Provider ID to check on claims and authorizations/referrals that were filed/created under the old Tax ID.
