1.  Why does my provider information not show up in the HMHS Provider Locator?
 Only HMHS TRICARE network providers are displayed in the HMHS Provider Locator. If you are unsure of your network status, you may contact your Provider Representative by calling 1-800-444-5445.

2.  My information shown in the HMHS Provider Locator is not correct. How do I get it corrected?
 The data shown in the HMHS Provider Locator is loaded daily from our Provider Information Management System. If the information is incorrect you can request that it be changed by contacting your local Tricare Provider Representative at 1-800-444-5445.
